GM Care Record

Community Pharmacy providers across Greater Manchester can now access a patient’s full medical record via the GM Care Record via PharmOutcomes, improving the quality and safety of care provided to patients in the community.

​​​How to request access to the GMCR

To request access to the GMCR visit the GMCR website and click the link to complete the digital GMCR Access Onboarding Form. 

Along with your pharmacy details, you will need to share the following information, so it’s recommended you have this to hand:

• ICO Registration Number and date of expiry - you can check this here.
• Data Protection & Security Toolkit status and date of submission. 
  
• External Internet Provider (IP) address. To locate this go to whatismyip.com.
  

Once submitted, your request will be processed and the relevant Information Governance process completed.
Once approved, notification and login instructions will be sent to your pharmacy’s nhs.net email address.
